Complaints with the Iowa Board of Behavioral Science or Iowa Board of Social Work against Treatment Providers, as well as other licensees and registrants of these boards, may be reported for investigation at the following address:

For all other patients, complaints against Treatment Providers may be reported for investigation to the appropriate licensing board of the state in which the patient received the services.
Lutheran Family Service recognizes its responsibility, as part of its Quality Assurance Program, to listen to client and applicant suggestions, complaints, or grievances, and attempt to resolve any such concerns. Clients or applicants who feel they have been treated unfairly by any member of Lutheran Family Service staff in providing services may file a grievance. Individuals filing grievances will be treated with dignity, understanding, and respect. In no case will a grievance result in any reprisal. Clients will not be denied service because of a grievance. A response can be expected within ten business days following the receipt of a formal grievance form.
